A tool for your health insurance

Get your fee quote for your health insurer

Select your procedure and receive your quote as a PDF, made out in your name, to send to your health insurer: a French complementary health insurer (“mutuelle”) or a private or international health insurer.

1. Your procedure

Prices for both eyes, as published on the prices page. For the ICL phakic implant, a quote is drawn up at the consultation.

2. Which eye?

3. Your details

Only what your health insurer expects to see on a quote. No medical information (medical history, examinations, treatments) is requested. Fields marked are required.

As shown on your insurance card or passport.

Your address appears on the quote, which can also be emailed to you as a PDF.

The quote covers the surgeon’s fees; the clinic package is shown for information. It is drawn up from the prices published on this site, before any consultation, and does not constitute a guarantee of reimbursement.

Why a quote for your health insurer?

Many policies include an allowance specifically for refractive surgery: LASIK, PRK, SMILE or PresbyLASIK. To tell you how much it will contribute, your health insurer usually asks for a quote in your name, issued in the surgeon’s name, showing the proposed technique and the amount of the fees. This is the document you get here, in a few moments. The quote is issued in French, the language of the practice, and the PDF includes an English courtesy translation on a second page, for insurers outside France.

The French state health insurance (Assurance Maladie), on the other hand, does not reimburse refractive surgery: “Refractive surgery is not covered by the Assurance Maladie. It is covered, often partially, by some complementary health insurers, depending on the policy taken out.” (ameli.fr, our translation)

What your quote contains

  • your identity (last name, first name, date of birth), as your health insurer knows it, and your email address;
  • the chosen technique, for both eyes;
  • the surgeon’s fees, which are the subject of the quote, with the CCAM code of the procedure (for SMILE, a reference code: the CCAM has no dedicated code for it);
  • for information, the clinic package (billed separately by the clinic where the procedure takes place) and the total cost of the procedure;
  • a unique number, the date of issue, a control code (your health insurer or the practice can verify the quote’s authenticity online) and the issuer’s professional identifiers (RPPS, SIRET and the Ordre des médecins council).

What the quote does not guarantee

The amount covered depends solely on the benefits of your policy: the quote does not constitute a guarantee of reimbursement, and only your health insurer can tell you how much it will contribute. As it is drawn up before any consultation, it is not a decision to operate either: the indication and the final technique are confirmed at the consultation, after an examination of your eyes.

What next?

  2. Your complementary health insuranceSend it the quote: only your insurer can state the amount it will contribute.
